Patient Forms & Policies
All new patients are required to complete the Cosmetic Hx Form prior to their first appointment. The Vagaro scheduling system will prompt you to complete these forms. You will also need to pay the Deposit to hold your appointment slot. On the day of your appointment, please bring a photo id and valid form of payment. We accept cash, check, Visa, Mastercard, Discover, American Express, Venmo, Zelle, Cherry (with fee) and Care Credit (with fee).

Pre Care Instructions
The results and longevity of each procedure is highly dependent on following the pre-treatment precautions and aftercare instructions. Dr. Ling will provide additional data for each procedure at the time of your consult to ensure you achieve your best results. Below are general precautions and after instructions for the most common procedures.
Special instructions for New Clients…please arrive early to apply numbing cream and complete the New Patient Intake Form prior to arrival. If you scheduled only an initial consultation, you may not have enough time designated for a treatment the day of your initial visit. If you want to add a procedure to your consultation, please check the add-on button for the requested procedure, at the time of booking. Please bring old photographs of yourself, preferably not smiling.
Special Instructions for injectable (Botox, dermal fillers, mesotherapy, threadlift, Agnes) procedures: please avoid alcoholic beverages or take Advil, aspirin, Aleve, excedrin, Ibuprofen, or Motrin, or fish oil for at least two days prior to your procedure. If you are prone to bruising, or if you drink regularly (1-2 drinks per week), please take Arnica tablets every four hours starting two days prior to the procedure, and continue two days after. Plan to not exercise for 24 hours after.
Special instructions for the Nouvaderm, CO2 or erbium fractional laser or Secret RF treatments: Please text or email if you are prone to cold sores / herpes, and the pharmacy phone # of your choice. Also include your birthdate. Please pick up your prescriptions, aquaphor, white vinegar (2 tsp), whole milk or plain yogurt, saran wrap, old t-shirts, and a large hat prior to the Nouvaderm, CO2, erbium, or Juliet lasers. Get all your public activities, like grocery shopping, out of the way, before you come in. **Arrive ONE HOUR early to self-numb** Plan for at least three days of staying inside for the CO2; 2 days for Nouvaderm.
Special instructions for those receiving the Juliet Vaginal Rejuvenation Laser: Please shave the outer vulvar area the night before. There is an additional charge if we have to shave you here. You must also have had normal Pap smears within the past year, and not currently be on your period. Arrive early to self-apply numbing cream to the outer area, and the first inch inside, and apply saran wrap before underwear (in the bathroom). *If you want to prenumb, please do not schedule a 9:00 appointment, as you may not have enough time before your procedure for adequate numbing.
Special instructions for those receiving Waxing / Sugaring services: Come in with 1/2-1 inch of hair growth (stop shaving 2 weeks prior). Avoid tweezing, depilatories, tan / sun exposure, exfoliation, retinoids, glycolic or salicylic acids, or Vitamin C for at least one week to the area.
